Resource Assistance Program (R.A.P.)

Not quite ready for hospice but need support services? Consider the Resource Assistance Program, R.A.P., offered by Hospice Promise.
R.A.P. is available free of charge to anyone that does not meet hospice criteria but needs resource assistance.
This is a non-medical Social Work program that offers coordination of care, and resource assistance to determine current and potential future needs.
R.A.P. is available for support for the individual and meet their needs through community-based services.
