Ann

Ann

Master of Social Work

I have been a social worker in the state of Arizona since 2007. I have a master’s degree in social work and a bachelor’s degree in Sociology with a concentration in Gerontology. Since moving to Arizona I have worked with the geriatric population in different environments; psychiatric, memory care, skilled rehab, long term rehab, home health and hospice. I have assisted the geriatric patient in every level of care and condition. However, after 11 years of working with this population my true passion is hospice care.  It is very rewarding and an honor as a social worker to assist the patients, their families and loved ones during the difficult journey through end of life.  As an MSW I can offer emotional support, the tools to cope with what is happening, community resources and bereavement support. I love what I do, and I work with an amazing team at Hospice Promise.

Samantha

Samantha

Transitional Coordinator

Before I joined this wonderful team, I was a licensed cosmetologist for 6 years. I started here with no hospice experience, but I quickly learned about hospice and really started to like what it was all about. It is so great to work with such an amazing hospice team!  I believe in this team so much that when my own grandmother needed hospice I knew exactly who I wanted to care for her. In my spare time I love being outdoors with my fiancé and two wonderful children.

Amber

Amber

RN Case Manager

I am a hockey mom to my very core and a Blackhawks fan for life. I joined the Hospice Promise team with the hopes of finding a more personal connection with my patients and their families than I would in a different setting. I’ve seen firsthand what the lack of hospice support looks like in the small town I grew up in. I’m proud to be a part of offering these services to such a special population.

Hospice Promise VA Representative
I am originally from Hampton Virginia. I am the 2nd oldest of 5. I have been around military service all my life. My father served for 24 years in the Army and I served 20 years in the Air Force. I have one daughter and a grandson. My hobbies and interests are watching sports and I love any activity involving the outdoors. I have been a member of the Hospice Promise team since December 28, 2017. I came to Hospice Promise through a very dear friend who informed me of her amazing hospice company and invited me to join the wonderful team.  I felt I couldn’t pass up the opportunity to join such a caring group of people, so here I am.

Patrick

Patrick

RN BSN Case Manager

I am a former Critical Care Nurse that has found a true passion for Hospice Nursing. Assisting patients and their loved ones through one of the most trying times in their lives is my true calling. I take great pride in knowing that the patient and their family are comfortable and peaceful. I have found a home here with Hospice Promise and work with a team that has a “patient first” mentality in which everyone is willing to assist in any possible way.  I have been married for almost 15 years and we have 2 boys, 15 years old and 11 years old. Our family is very close, and we are involved in youth sports in the surprise community. We are also animal lovers, with 4 dogs and 1 cat. We spend most of our free time enjoying our family and being “home bodies.” My wife is an RN as well with a background in the Emergency Department and Corrections Nursing.

girl

Andrea

Certified Nursing Assistant

I am from the Dine Nation and have been with Hospice Promise since September 2015. I am married and have a 25-year-old son. Working in hospice feels like it suits my soul, something at the very nature and essence of my being. People always ask me what I do and how I deal with all of the emotions. I deal with the emotions by providing comfort, reassurance and stability to my patients and their families during a very difficult time. Something I have realized during my work in hospice is that, to have an interest in death/dying does not imply a morbid fascination with the end of life. To me, it suggests a deep reverence for the life we are lucky to live, even for a short time. I always tell myself that ”I assist the patient and their families through the process and to do so with dignity.” I feel so blessed to be a part of a team that gives themselves to their work, that respects their patients and families, and that above all, honors life. Aho!
