Resource Assistance Program

Hospice Promise’s Resource Assistance Program (RAP) offers coordination of care, support and resources to individuals facing a life-limiting illness, catastrophic disability or frailty associated with chronic disease, aging or injury, or those who will be stepping down from hospitalizations to less intensive care.

Our team of experts will determine current and potential needs and see how these can be met through community-based services. Indoing so, our goal is to enhance quality of life by working together with patients, their families and other health care providers to offer an extra layer of support and education. Our sole focus is to help patients and their families manage their changing needs and provide relief from physical and emotional pain. At Hospice Promise, we treat the patient as a “whole” rather than focusing on the diagnosis alone.
